• Poka-Yoke – Mistake Proofing
• Workshop on Team exercises using Poka-Yoke techniques

Session 2: Understanding Corrective Action
• The corrective-action process
• Understanding problem identification and brain storming
• 5 Why’s techniques and root cause analysis
• Analyzing effectiveness of their implementation
• 5W1H and 6W2H
• Because Logical Analysis Techniques
• Detailed specification of the problem
• Documentation of the background information and evidence
• Brainstorming of the causes
• Short listing of the causes
• Further detailed analysis of the short list using the 5 Why’s technique
• Conclusion as to the most likely root cause(s) of the problem
• Understanding Preventive Actions
